Patient Forms

To Help You Prepare For Your First Appointment

When you call for a New Patient Appointment at Lesch Family Dental, you will be scheduled for a 1 hour appointment. We will take the necessary x-rays and complete a comprehensive periodontal and dental charting exam.

Your treatment needs will be then be discussed with you by Dr. Lesch and a custom, comprehensive plan will be designed to give you the beautiful, healthy smile that you deserve.

For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards.

Financial Policy:

It is important that you, as the patient, understand that our credit and collection policies are a necessary part of assuring the financial resources required to maintain this vital health care service for our patients and the community.

Payment for services is due at the time services are rendered.

If you have dental insurance, it should be understood that this is a contract between you, your employer, and the insurance company. Our office, therefore, can never guarantee what any insurance company will pay for or how much they will pay. Your dental bill is an agreement between you and your dentist. You are responsible for the payment of your bill regardless of the status of your insurance claim.

Insurance Policy:

Some dental practices file insurance for their patients and some do not. As a courtesy and convenience for you, our practice does file insurance.

It is our responsibility to file as efficiently as possible for you to ensure the maximum possible benefit. We will also predetermine insurance benefits for given treatments when required.

To do the best job filing insurance, we need to have our patients provide us with accurate insurance information.
